The two systems are not mutually exclusive: you can remain entitled to the SNS and still use private care when you want faster access, a particular specialist, more appointment choice or a specific private hospital.

This guide is about how to use private healthcare in practice. For a broader comparison of health, home, motor, liability and other insurance, see our internal Insurance in Portugal guide. For public referrals and specialist access, see Hospitals, referrals and specialists in Portugal.

Private healthcare does not replace the SNS

Buying private health insurance does not normally cancel your SNS entitlement. Many people use both: public primary care, vaccination and emergency pathways through SNS, while using private insurance for faster specialist appointments, diagnostic tests or planned treatment.

If you have an immediate life-threatening emergency, use 112. Private insurance is not a substitute for emergency routing.

Why people choose private care

Quicker access to a specialist or diagnostic test.

Direct booking without first obtaining an SNS referral.

Choice of doctor, clinic or hospital.

More convenient appointment times or locations.

Access to a private hospital network close to home.

A second opinion while remaining in the SNS for other care.

These are practical reasons, not guarantees. Availability depends on specialty, location, insurer network and your policy.

The major private hospital networks

Large private groups make it easy to search doctors, specialties and locations online. Common examples include:

CUF

Hospital da Luz

Lusíadas Saúde

HPA Saúde, especially relevant in the Algarve and some other regions

These are examples, not rankings. Portugal also has many independent private clinics, diagnostic centres and specialist practices. Use the ERS provider search to check whether a healthcare establishment is registered.

Using private care without insurance

You can simply pay privately. This can make sense for an occasional consultation or if the annual premium for insurance would cost more than the private care you expect to use.

Before the appointment, ask for the self-pay price. A consultation price normally does not automatically include blood tests, imaging, procedures, materials or follow-up. Ask for a written estimate before a planned procedure or admission.

Health insurance: network, reimbursement or both

Portuguese health-insurance products commonly work through one or both of these mechanisms:

Network/direct settlement: you use a doctor or hospital in the insurer’s network and pay the contracted co-payment or deductible.

Reimbursement: you pay the provider and submit the invoice to the insurer, which reimburses an eligible percentage up to the policy limits.

Do not assume that a hospital displaying your insurer’s name means every doctor, specialty and procedure is covered. Hospital da Luz, for example, explicitly advises patients to verify the specific service with the insurer, and CUF similarly warns that not every medical act is covered by every agreement.

Common insurers and healthcare networks

Common Portuguese health-insurance brands and networks include Multicare, Médis, Allianz, and products distributed by other major insurers. Provider networks such as AdvanceCare also appear behind a number of insurance products.

Compare the actual policy, not only the brand. Check the ASF authorised-entity register before buying from an unfamiliar insurer or intermediary.

What to compare before buying

Hospitalisation limit: the annual amount available for admissions/surgery.

Outpatient care: specialist consultations, tests and treatments.

Co-payments: what you pay each time you use the network.

Deductible/franquia: the amount you must bear before some cover applies.

Reimbursement percentage and limits: particularly if you want doctors outside the network.

Network near your home: a large national network is less useful if your preferred hospital or specialist is excluded.

Dental, maternity, mental-health and physiotherapy cover: these may have separate limits or exclusions.

Medication and optical benefits: often limited or excluded.

Age limits and premium increases: premiums commonly change with age.

Waiting periods: cover may not begin immediately.

Pre-existing conditions and waiting periods

Do not hide existing medical conditions on an application. ASF explains that a known pre-existing condition is covered only if it has not been expressly excluded by the contract; in practice, exclusions are common. A contract can also impose a waiting period of up to one year for cover of pre-existing conditions.

Read the health declaration and policy wording carefully. An inexpensive policy that excludes the condition you are most likely to need treated may provide little value for that problem.

Insurance versus a health discount plan

A health plan or discount card is not automatically the same thing as health insurance. A plan may simply give access to negotiated prices within a network, while true insurance transfers specified financial risk to an authorised insurer subject to the contract.

Always ask: Is this insurance, or just a discount/access plan? If it is insurance, identify the authorised insurer and check it in the ASF register.

How to use your insurance for a consultation

Search your insurer’s provider network for the specialty and location.

Confirm that the specific doctor and hospital/clinic participate.

Ask whether prior authorisation is required.

Check your co-payment or deductible.

Take your ID and insurance details to the appointment.

Before tests or procedures, verify that they are also covered; consultation coverage does not automatically mean every test is covered.

Keep invoices, medical reports and authorisation references.

Pre-authorisation matters

Hospital admissions, surgery, expensive imaging or other planned procedures may require insurer authorisation. Do not assume a doctor’s recommendation equals insurer approval. Ask the provider to send the clinical request/estimate and obtain confirmation before treatment where possible.

If you want to use a doctor outside the network

Check whether your policy includes reimbursement. If it does, ask:

What percentage is reimbursed?

What is the maximum eligible amount?

Is there an annual cap?

What documentation must be submitted?

How long do you have to submit the claim?

If there is no reimbursement cover, an out-of-network appointment may simply be self-pay.

Private emergency and urgent care

Some private hospitals have 24-hour or extended-hours urgent-care services. Insurance coverage and co-payments can differ from ordinary consultations. For a serious or life-threatening emergency, use 112; do not delay emergency assistance while checking a private insurance network.

Pregnancy, maternity and planned procedures

Maternity cover often has significant waiting periods. If pregnancy or another foreseeable planned treatment is an important reason for buying insurance, verify eligibility before purchasing. Do not assume a policy bought after treatment becomes foreseeable will cover it.

Moving between SNS and private care

Keep copies of relevant reports, test results and your medication list. Public and private systems do not necessarily share every record automatically. If a private specialist recommends treatment that you want through SNS, take the report to your SNS clinician; the SNS still follows its own clinical and referral process.

Can a private doctor prescribe medicines?

Yes, appropriately licensed doctors can prescribe. However, reimbursement/subsidy and follow-up can depend on the medicine, prescription and clinical context. Keep the prescription and report if ongoing treatment will later be managed by your SNS doctor.

Choosing a broker or adviser

An insurance intermediary can be useful if you want help comparing networks, exclusions and family policies. Verify the intermediary in the ASF register. Ask whether the intermediary represents one insurer, several insurers, or provides broader advice, and how they are remunerated.

For a broader explanation of intermediaries, home, liability and other policies, use our Insurance in Portugal guide.

Complaints and checking providers

Healthcare providers are regulated by the Entidade Reguladora da Saúde (ERS). ERS maintains a public register of healthcare establishments and deals with patient-rights/provider issues. Insurance companies and intermediaries are supervised by the Autoridade de Supervisão de Seguros e Fundos de Pensões (ASF).

A practical way to decide whether insurance is worth it

Compare the annual premium with what you actually value:

Do you mainly want two or three quicker specialist consultations a year?

Do you want protection against a large private hospital bill?

Is your preferred local hospital in the network?

Are the conditions you care about excluded?

Would self-paying occasional appointments be cheaper?

Do you need maternity, dental, mental-health or physiotherapy cover?

There is no universally best answer. A policy can be valuable for convenience and financial protection, while another person may prefer SNS plus occasional self-paid private consultations.

Useful links

ASF — authorised insurers and intermediaries

ERS — search registered healthcare providers

CUF — hospitals and clinics

Hospital da Luz

Lusíadas Saúde

Multicare

Médis

BorderlessAdvice — Insurance in Portugal

BorderlessAdvice — Hospitals, referrals and specialists

Last verified: 2 October 2026.
