In private care you can often book a specialist directly, but you should check the price, insurer/network rules and whether the provider is registered before the appointment.

Quick rule: emergency now → 112 or the appropriate emergency route. New non-emergency specialist problem in the SNS → normally start with primary care/referral. Want to use private care → you can often contact the private hospital or specialist directly, but confirm insurance authorisation and your own cost first.

Public hospitals, private hospitals and ULS: the basic map

Portugal’s mainland SNS is organised largely through Unidades Locais de Saúde (ULS), which integrate primary and hospital care in a geographic area. The ERS reported 39 ULS in its 2026 study. This means your health centre and public hospital are increasingly parts of the same local system rather than completely separate worlds.

Private hospitals and clinics operate alongside the SNS. They may be used by self-paying patients, people with private health insurance, people using a health plan, or—in some services—under agreements with public or other financing systems. Always confirm the exact arrangement for the specialty/service you need.

How do I see a specialist through the SNS?

For most non-emergency first specialist appointments, you do not simply choose any public hospital consultant and book directly. A clinician assesses the problem and refers you when specialist hospital care is indicated. Your family doctor can be an important route, but being sem médico de família does not mean specialist referral is impossible: use the primary-care access arrangements explained in our family-doctor guide and ask the health unit how an unassigned patient can be assessed.

Seek assessment through your health centre/primary-care route.

The clinician decides whether hospital/specialist referral is clinically indicated.

The referral enters the relevant SNS pathway.

The hospital/service processes the request and schedules according to clinical priority and capacity.

Attend with your identification, health information, medication list and requested test/results.

After the first consultation, the hospital specialty normally manages necessary follow-up within that episode, while primary care continues your general care.

The official gov.pt hospital-consultation service explains hospital consultations and links the applicable guaranteed-response-time framework.

What if I do not have a family doctor?

Do not interpret “no assigned family doctor” as “no referrals.” Ask your registered health centre how it provides medical assessment to patients without an assigned doctor. Depending on the unit and clinical situation this may be through another doctor/team, an open consultation or another local arrangement. See our Family doctor in Portugal — and what if you don’t have one guide.

Can I choose the public hospital or specialist?

SNS referrals are governed by the public system’s referral, clinical-priority and capacity arrangements. Do not assume that a referral gives unrestricted choice of any named consultant. If location, a particular hospital or continuity with an existing specialist matters, raise that with the referring clinician and ask what choices are available in your case.

Waiting times and priority

Waiting time is not one universal national number. It varies by specialty, hospital/ULS, clinical priority and type of procedure. Portugal has Tempos Máximos de Resposta Garantidos (TMRG) for various non-urgent healthcare services, and the official hospital-consultation page links that framework. If a wait appears excessive, ask the hospital/ULS what priority was assigned, what the applicable access rule is and whether an alternative pathway is available.

ERS’s 2026 ULS study found meaningful variation between ULS in access, resources, capacity and compliance with guaranteed response times, which is why a national anecdote about “the waiting time” can be misleading.

Tests, scans and other diagnostics

A specialist or primary-care clinician may request blood tests, imaging or other diagnostics. Some are performed inside an SNS hospital; others can be performed by providers operating under an SNS convention. Check the referral/prescription, where it can be used, any validity period and whether the provider has the relevant convention for that exact service.

Private specialists: often much more direct

Private hospitals and clinics commonly allow patients to search by specialty and book directly. This can be useful if you want a faster appointment, a specific clinician, a second opinion or more scheduling choice. But “private” does not mean a single fixed price: consultation, tests, procedures, hospital fees and follow-up can all be separate.

Before booking, ask:

What is the self-pay consultation price?

Is the doctor and this location in my insurer’s network?

Do I need insurer pre-authorisation?

What is my co-payment/deductible?

Are tests performed during the visit charged separately?

Will a proposed procedure require a separate estimate/authorisation?

Health insurance: use our insurance guide

If you are considering private care because of waiting times, direct specialist access or provider choice, see our internal Insurance in Portugal guide. It explains Portuguese health insurance, the difference between real insurance and health discount plans, exclusions, waiting periods, networks, reimbursement arrangements and how to check insurers/intermediaries.

Important: having a health-insurance card does not mean every doctor at every private hospital is covered. ERS advises patients to confirm whether the provider has an agreement with their insurer for the specific specialty and service they want.

Finding and checking a hospital, clinic or specialist

The ERS provider search is particularly useful because healthcare establishments in mainland Portugal across the public, private, cooperative and social sectors are subject to ERS registration. ERS also recommends checking that the individual health professional has a valid professional credential from the competent professional body.

For public services, use the official Portuguese health-unit finder. For hospital quality information, ERS also operates SINAS; read its methodology rather than treating a symbol or classification as a universal ranking of doctors.

Private hospital groups

Large private networks can be useful for finding specialists and checking availability. Examples include CUF, Hospital da Luz, Lusíadas Saúde and HPA Saúde. These are examples, not a ranking or endorsement. Check ERS registration, specialty availability, insurer network and price for your own appointment.

Second opinions and moving between public and private care

You can have records from both sectors. If you obtain a private diagnosis or test, bring the relevant report/results to your SNS clinician rather than assuming the systems automatically share everything. A private specialist’s recommendation does not necessarily bypass the SNS referral/clinical-decision process for public treatment. Conversely, you can take copies of relevant SNS information to a private specialist.

Prescriptions and follow-up

Clarify who is responsible for ongoing prescriptions and monitoring. A hospital specialist may prescribe within the episode, but stable long-term care can later involve primary care. If a private specialist starts treatment, make sure you understand how prescriptions, repeat monitoring and any transition to SNS care will work.

When not to wait for a specialist appointment

If symptoms become an immediate emergency, do not wait for a scheduled consultation or referral. Call 112 when there is an immediate threat to life or serious emergency. For urgent symptoms where you are unsure what level of care is appropriate, call SNS 24 on 808 24 24 24. See our SNS24, emergencies and when to use 112 guide.

Problems, bills and complaints

For private care, request price information before non-emergency treatment and keep estimates, invoices and insurer authorisations. If there is a problem with access, billing, information or a healthcare establishment, the Entidade Reguladora da Saúde (ERS) regulates public, private, cooperative and social healthcare establishments and provides patient-rights and complaint information.

Practical checklist

Emergency? Use 112; unsure about urgent symptoms? SNS 24.

For ordinary SNS specialist care, start with clinical assessment/referral.

No family doctor? Ask your health centre how unassigned patients obtain assessment and referral.

Keep your contact details current so the hospital can reach you.

Ask about the specialty, hospital/ULS and clinical priority if the wait is unclear.

For private care, confirm price and insurance authorisation before treatment.

Verify providers through ERS.

Carry/share relevant reports and medication lists when moving between public and private care.

Use our Insurance in Portugal guide before choosing private health cover.

Useful links

gov.pt — hospital medical consultations

gov.pt — find health units

ERS — search registered healthcare providers

ERS — SINAS health-provider quality information

ERS — patient rights and complaints

SNS 24

Last verified: 2 October 2026.
