It is not private travel insurance.

EU resident in Portugal but another EU state remains responsible for your healthcare: an S1 may be the relevant route; register it in Portugal.

Travelling specifically for planned treatment: this is different from an EHIC visit and may require S2/prior authorisation.

Emergency: call 112. For non-emergency triage/advice call SNS 24: 808 24 24 24.

Is the SNS free?

The SNS is tax-funded public healthcare and many services are provided without a patient charge. Portugal removed most antigas taxas moderadoras, including routine primary-care consultations and prescribed diagnostic tests, but do not interpret this as a promise that every possible service, medicine, dental treatment, device, private provider or situation costs nothing. Prescription medicines can involve patient co-payments and private care follows private/insurance pricing.

When in doubt about your own entitlement or a requested charge, ask the SNS unit to check your registration rather than assuming that possession of a número de utente settles the question.

EU citizen: visitor, resident or S1?

Your situation

Typical route

What it means

Temporary visit to Portugal

EHIC / CESD

Medically necessary state healthcare during the temporary stay, under the applicable public-system conditions.

You move to Portugal and Portugal becomes responsible for your healthcare

Portuguese SNS registration

Register your Portuguese entitlement and use the SNS as a resident; do not treat an EHIC from your old country as permanent resident cover.

You live in Portugal but another EU country remains responsible

S1

Obtain the S1 from the competent institution and register it in Portugal. This can arise in pensioner, posted-worker and other coordinated cross-border situations.

You travel to Portugal specifically for planned treatment

S2 / prior-authorisation rules may apply

Planned cross-border treatment is not the same thing as unexpected/necessary care during a visit.

Example: a Dutch, Belgian, German or French tourist who remains insured in their home country would normally use a valid EHIC/CESD for necessary public care during a holiday. If that same person genuinely relocates to Portugal, the correct resident route depends on which country’s system is responsible for their healthcare. EU coordination is based on status and applicable social-security rules, not simply passport nationality.

gov.pt — medical care in Portugal for European citizens explains EHIC, S1 and S2 routes. For the wider EU rules see Your Europe — health cover when living abroad.

Getting a número de utente

The número nacional de utente identifies you in the SNS. Foreign nationals can be assigned one when they first use a public health unit, and it can also be requested without waiting until you are ill. The number itself is free.

Important: the Portuguese government explicitly states that receiving a número de utente does not by itself guarantee that the SNS covers the cost of care. Your record needs the information supporting your entitlement. Current government guidance for foreign residents identifies information such as identity, Portuguese NIF, Portuguese address and valid residence documentation, subject to legal exceptions.

gov.pt — obtain an SNS user number. Eligible foreign residents can also check the newer combined NIF + NISS + SNS-number service, currently offered at participating Espaços Cidadão.

Register with a centro de saúde

Once your record is in order, register with a centro de saúde, preferably the one serving your residential area. Primary care includes general/family medicine, nursing and other community services. Having registered does not necessarily mean that a named médico de família is immediately available; allocation depends on local capacity.

gov.pt — register at a health centre. Guide #78 goes deeper into centros de saúde/USF and guide #79 covers what to do if you do not have a family doctor.

SNS 24, urgent care and 112

For illness that is not an immediate life-threatening emergency, SNS 24 (808 24 24 24) provides triage, advice and referral. For a serious or life-threatening emergency call 112. Do not use 112 as a general medical-advice line.

SNS 24 also provides digital health information/services. Guide #80 will cover SNS24, urgent care and emergencies in detail.

Hospitals and specialists

The public system is not generally designed as “choose any specialist and book directly” in the same way as many private networks. Non-emergency specialist care commonly follows referral pathways. Emergency departments are for urgent problems; SNS24 can help direct you to the appropriate level of care.

Medicines and pharmacies

Portugal has a broad pharmacy network. SNS prescriptions can qualify for public reimbursement according to the medicine and patient’s circumstances, which means the patient may pay only part of the retail price. Do not assume that “SNS is free” means all prescribed medicines are free. Guide #83 covers pharmacies and medicines in detail.

Why do people still buy private health insurance?

Private health insurance is normally supplementary for someone who already has SNS rights, not a replacement for the public system. Common reasons include faster access for some consultations/tests, direct access to private specialists, choice of private hospitals/clinics, convenience and predictable network prices. Waiting times and access vary by specialty and location, so the value is personal.

Before buying, compare waiting periods (períodos de carência), exclusions/pre-existing conditions, annual limits, hospitalisation limits, outpatient cover, co-payments, reimbursement versus network-only treatment, age-related pricing and cancellation/renewal rules.

Insurance is not the same as a health discount plan

A seguro de saúde is insurance. A plano de saúde can instead be a discount/access arrangement with participating providers. It may look cheaper because it is not providing the same insured risk. Read the contract rather than comparing only the monthly price.

Where to compare private options

Examples of large insurers/brands active in Portugal include Fidelidade, Ageas, Allianz, Generali Tranquilidade, Médis and Multicare. These are examples, not endorsements.

Private hospital groups include CUF, Lusíadas Saúde and Hospital da Luz. Check whether the particular hospital, doctor and treatment are actually in your insurer’s network.

If using a broker/mediator, verify the firm or person through the ASF authorised-entities register. ASF is Portugal’s insurance and pension-fund supervisor.

A practical first-week healthcare checklist

Work out which country/system is responsible for your healthcare; EU nationality alone is not the answer.

If visiting from another European system, carry/check your EHIC/CESD and separate travel cover.

If relocating, obtain or confirm your número de utente and make sure the entitlement information attached to it is correct.

If an S1 applies, obtain it from the competent foreign institution and register it in Portugal.

Register at the appropriate centro de saúde.

Save SNS24 808 24 24 24 and 112.

Only then decide whether private insurance adds enough convenience/access for your circumstances.

Useful official links

gov.pt — healthcare in Portugal for migrants

gov.pt — healthcare for European citizens

gov.pt — obtain número de utente

gov.pt — register at a health centre

SNS 24

Your Europe — healthcare cover when living abroad

ASF — authorised insurers and intermediaries

Last verified: 2 October 2026.
